1. I will read and uphold the Mount Forest Minor Hockey Rules of Operation.

2. I will comply with all rules and regulations of the OWHA, OWHL, WOGHL, WOAA, and any other governing bodies.

3. I will complete my Police Vulnerable Sector Check before July 2026.

4. I will upgrade my coaching credentials as required.
