Below is a link to the OHF Screening questionnaire that families MUST review before coming into the arena.
This is extremely important to complete, the questionnaire is confusing and not ideal, however it is what we have to comply with at this time.
If we find this is not being done, we will have to implement a more tedious screening at the door, which will take us longer to check kids in.
Please do your part, it does not take a lot of time to review this once you understand it.
Do not come to the arena if you are UNWELL, or if you have been around a family member that is UNWELL.
We cannot stress this enough.
